Do you or your family need more Medical Resources and information?

Included on this site is a list of free or low-cost clinics in the Chicago area – a valuable resource for anyone searching for inexpensive care.

Emergency Contact Database Now Available

The Secretary of State's Office now offers an online database for emergency contacts. Anyone with an Illinois driver's license, permit, or state ID can taaake advantage of this new service. The database allows each cardholder to enter two emergency contacts online. In case of an emergency, where the participant is unable to speak to emergency responders, law enforcement can access to the information to reach your emergency contacts by using your ID.
 
This voluntary program will make it much easier to reach family and friends in emergencies. To sign up and find out more information click here.

Do you want to prevent unwanted telemarketing calls to your cell phone?

As of January 24, 2009 cell phone numbers have been released to telemarketing companies. Please note that you may be charged for these calls. To block these calls, call the National Do Not Call list from your cell phone at 888-382-1222. This will block your cell phone number for five (5) years. You MUST cal l from the cell phone number you wish to have blocked, and not from another number. You can also register you phone numbers at the National Do Not Call List website.

Assistance with Federal, State, and Local Agencies

Before my staff can assist you with other agencies, we must have your written permission. This is to protect your privacy rights.

You can download the Privacy Release Form here to view and print.

Once you have completed and signed the Privacy Release Form, you may mail, fax, or personally deliver it to my district office.

Often you can find much of the information you need from various agency websites without having to come in or call. Our State Agencies page lists many of the Illinois websites available to you.
